Rural health network hospitals’ grant to aid those with chronic illness

In 2014, 10 Maine hospital chief executive officers gathered to work on an opportunity to secure a federal grant to support chronic disease management in rural Maine. In early 2015, the group was notified that it had been awarded the Maine Rural Health Innovations Network Grant.
